A Case of this week
Diagnosis:
Rt. Age related Macular Degeneration
(AMD), macular drusen, Lt. Wet AMD, Submacular haemorrhage, VA 6/18, HM cc, IOP
17,19 mmhg Fundus Photography ReportRight
and left eye coloured and fluoroscein fundus photography were done. Enclosed
are the photographs. N.B.
Photos on the right hand side of the report are those of the right eye and those
on the left hand side are those of the left eye. Coloured photos: -
Right eye shows numerous small macular and parafoveal flat discrete yellow
deposits at the level of Bruch’s membrane (Hard Drusen), many larger
parafoveal slightly raised and less distinct yellow deposits (Soft Drusen). -
Left eye shows numerous Macular and foveal large soft drusen coalesced with the
presence of pigments clumps on the drusen (Drusenoid pigment epithelium
detachment) which indicates chronicity.
Subretinal elevation involving all tha macula including the fovea,
surrounded by an area of serous retinal detachment, with pale and raised
appearance and a cosiderable amount of lipids and subretinal haemorrhage.
Right eye shows early hyperfluorescent spots corresponding to the hard drusen,
early hypofluorescent spots that some of which become fluorescent late (soft
drusen), many parafoveal sharply defined round and oval patches of early and
persistent hyperfluorescence window defects due to (Geographical atrophy) of
retinal pigment epithelium.
Left eye shows early central hyperfluorescence of leakage of the dye corresponding to the choroidal neovascularization. Its true extent can not be shown because of the masking hamorrahge appearing as a black area (blocked fluorescence). Dr Osama Al-Nahrawy
